Cal Fresh Assistance:

Cal Fresh, also known as the Supplemental Nutrition Assistance Program (SNAP), provides eligible individuals and families with financial assistance to purchase nutritious food. Our team offers guidance on the application process, eligibility requirements, and helps you maximize your benefits to ensure you have access to healthy meals.

Enhance Care Management: How it Works?

Qualified applicants will be matched with a Care Manager, who will

Help to reach life

goals, such as finding

housing, getting a job,

and accessing food

Coordinate care and

services by keeping

Support learning by

answering questions

about health conditions,

medications, and

accessing resources

Who is Eligible?

Medi-cal recipients must meet two criteria to be

eligible for Enhanced Care Management

Must fit into one of more of the following populations:

Experiencing homelessness, at risk of homelessness, or fleeing domestic violence

○ High emergency department use

○ Severe mental illness

○ Substance abuse disorder

○ Adults Living in the Community and at Risk for LTC Institutionalization

○ Adults Nursing Facility Residents Transitioning to the Community

○ Intellectual or developmental disabilities (I/DD)

○ Transitioning from incarceration

○ Prenatal or postpartum

○ Children enrolled in CCS or Child Welfare

initial screening,

your application will

3. A Pair Team

representative will

contact you in 1-2
